Online complaint forms

 

Complaints can be lodged using one of our on-line complaint forms.  There are two different forms available:

1.   Form for complaints about government agencies

Use this form if you wish to make a complaint about the actions or decisions of a central or local government agency.  For more detailed information about the types of complaints an Ombudsman can investigate, see What we do - complaints about government agencies.

2.  Form for official information complaints

Use this form if you want to make a complaint about a response you have received to an official information complaint.  For more detailed information about the types of complaints an Ombudsman can investigate, see What we do - official information complaints.

NOTE

These forms are not sent over a secure connection and may be intercepted by others.  If the information you wish to send is sensitive, we advise you not to use these forms or email as a way to contact us.

Any information received by an Ombudsman through this form will not be made publicly available.
